Hydrocodone, Ibuprofen falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Propolis interacts with.
In vitro research shows that propolis extract can inhibit CYP2D6. However, animal research shows that propolis extract does not significantly affect CYP2D6 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2D6 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing about Vicoprofen (hydrocodone plus ibuprofen): the hydrocodone part is actually a prodrug. Your liver uses an enzyme called CYP2D6 to convert it into hydromorphone, which does much of the pain-relieving work. So if propolis slowed that enzyme down, you'd expect less conversion and possibly a bit less pain relief, not more.
But honestly, I wouldn't lose sleep over this one. The only signal here comes from test-tube studies, and even animal studies didn't show a real effect, likely because very little propolis makes it into the bloodstream intact. It's never been reported in people. Normal propolis use is unlikely to change how your medicine works. Still, if you take concentrated doses regularly, mention it to your pharmacist or doctor.

Vicoprofen pairs hydrocodone (a painkiller) with ibuprofen, and it's the ibuprofen half that matters here. Your liver uses an enzyme called CYP2C9 to clear ibuprofen out of your body. In test-tube studies, propolis seems to slow that enzyme down, which in theory could let ibuprofen build up a little and bring on more of its usual side effects, like stomach upset, heartburn, or, with bigger amounts over time, irritation or bleeding in the gut.
Here's the reassuring part: this is only seen in lab dishes. When propolis was given to live animals, it didn't meaningfully change this enzyme, likely because so little gets absorbed. It's never been reported in people. Normal propolis use is unlikely to cause trouble, but if you take high-dose supplements, just mention it to your pharmacist or doctor.

Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the situation with Vicoprofen, which combines hydrocodone (an opioid) and ibuprofen (an anti-inflammatory). The hydrocodone piece is partly handled by a liver enzyme called CYP3A4. In test-tube studies, some propolis compounds appear to slow that enzyme down. If that actually happened in your body, the hydrocodone could linger and build up a bit, which in theory might mean more drowsiness, dizziness, constipation, or slowed breathing.
But here's the reassuring part: this is very weak, lab-only evidence. Animal studies and the way propolis is poorly absorbed suggest it likely doesn't meaningfully affect this enzyme in real people, and no human cases have been reported. I wouldn't lose sleep over it, but it's always smart to mention any supplements to your pharmacist or doctor.

ANTICOAGULANT/ANTIPLATELET DRUGS

In vitro research shows that propolis water extract and the propolis constituent, caffeic acid phenethyl ester, can inhibit platelet aggregation. Additionally, evidence from an animal model shows that taking propolis in addition to warfarin decreases INR, suggesting that propolis can decrease the effectiveness of warfarin.

Here's the thing to keep in mind with Vicoprofen: it's a combo of hydrocodone (an opioid for pain) and ibuprofen (an NSAID). The ibuprofen piece is what matters here. Ibuprofen already has a mild blood-thinning effect, and propolis appears to do something similar in lab studies, where it can slow platelets from clumping. Put them together and you could, in theory, nudge up your risk of bleeding or bruising.
Now, I want to be honest about how soft this evidence is. It comes from test-tube and animal research, not real-world cases, so it may not amount to much for most folks. Still, watch for easy bruising, nosebleeds, or stomach upset, and check with your pharmacist before adding concentrated propolis supplements.
