Why this applies to Macitentan, Tadalafil

Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP2C9.
In vitro research shows that propolis extract can inhibit CYP2C9. However, animal research shows that propolis extract does not significantly affect CYP2C9 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2C9 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Opsynvi combines two blood pressure lowering medicines: macitentan (an endothelin receptor blocker) and tadalafil (the same active ingredient as Cialis). The concern here is a lab-only one. In test tube studies, propolis can slow down an enzyme called CYP2C9, which helps clear these drugs. If that happened in your body, the drugs could build up a bit and lead to more pronounced effects like low blood pressure, dizziness, flushing, headache, or a stuffy nose.

Here's the reassuring part: this has not been seen in people, and even animal studies didn't show a real effect. Everyday amounts of propolis are unlikely to cause trouble. Still, since Opsynvi affects your blood pressure, it's worth a quick chat with your pharmacist before adding concentrated propolis supplements.

Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. Opsynvi combines two medicines, macitentan and tadalafil, and both are cleared by a liver enzyme called CYP3A4. The idea here is that some lab (test tube) studies hint propolis might slow that enzyme down. If it did, these drugs could build up a bit, which for this combo could mean more low blood pressure, headache, flushing, stuffy nose, dizziness, or swelling in the legs.

Here's the honest part: this is theoretical and based only on test tube and animal work, and it has never actually been reported in people. Other studies show no effect at all. Normal food-style amounts almost certainly don't matter, and even concentrated supplements may not either. Still, since this is a serious heart and lung medication, run any propolis product by your pharmacist or doctor first.
