Why this applies to Riociguat
Riociguat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body clears riociguat (Adempas) using a liver enzyme called CYP3A4. In a test tube, some propolis compounds appear to slow that enzyme down. If that happened in real life, riociguat could build up and hit harder, which might mean a bigger drop in blood pressure, leaving you dizzy, lightheaded, or even faint. Riociguat isn't a prodrug, so more drug would mean stronger effects, not weaker ones.
The reassuring part: this is only lab and animal data, and other studies showed no effect at all. It hasn't been seen in people, and food-level amounts are unlikely to matter. Still, since riociguat affects blood pressure, it's worth a quick word with your pharmacist or doctor before adding a concentrated propolis supplement.

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. Early research suggests it may help with cold sores, mouth sores, and minor wounds, but high-quality evidence is limited for most uses. It is generally well tolerated, but allergic reactions can occur, especially in people allergic to bee products.

What Is Riociguat Used For?

Riociguat — sold as Adempas and under the generic name Riociguat — is used to treat adults with two conditions that cause dangerously high blood pressure in the arteries of the lungs.
- Chronic Thromboembolic Pulmonary Hypertension (CTEPH): Used after surgical treatment, or when surgery is not an option, to improve exercise capacity and functional class.
- Pulmonary Arterial Hypertension (PAH): Used to improve exercise capacity, functional class, and to delay the condition from getting worse. It can be used alone or combined with certain other PAH medicines.
