Here's the gist with sage and your buprenorphine (Belbuca). Buprenorphine is a pain medicine that already has a calming, sedating side to it. Some natural compounds in sage are thought to have their own mild sedative activity. So in theory, pairing the two could add up and leave you feeling more drowsy, dizzy, foggy, or relaxed than usual. Buprenorphine isn't a prodrug here, so this is simply about two things that calm the nervous system possibly stacking together.

I'll be honest though, this concern is based on lab and animal data, not real patient reports, so it's pretty theoretical. Cooking with sage is very unlikely to matter. Concentrated sage extracts or the essential oil are where I'd be more cautious. If you're using one of those, just run it by your pharmacist or doctor first.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Here's the gist for your Belbuca (buprenorphine). Your body relies on a liver enzyme called CYP3A4 to help break this medication down. In test-tube studies, concentrated sage extracts seemed to slow that enzyme. If that held true in people, buprenorphine could clear a little more slowly and build up, which might mean stronger effects like extra drowsiness, dizziness, nausea, constipation, or slower breathing.

I want to be straight with you, though: this is only theoretical, based on lab work, and has never actually been reported in real patients. The sage you sprinkle on food is very unlikely to matter. Concentrated capsules or the essential oil are where I'd be more cautious. Before adding a sage supplement, check with your pharmacist or doctor, since buprenorphine is a medication we like to watch closely.
