Here's the gist with Hycodan or Tussigon, which pairs hydrocodone (an opioid) with homatropine. Hydrocodone already slows down your brain and breathing, and it can leave you drowsy, lightheaded, or foggy. Some compounds in sage appear, at least in lab and animal testing, to have a mild calming or sedating effect of their own. So the worry is simple: stacking the two could leave you sleepier or more out of it than expected.
I want to be honest that this is theoretical and not proven in people, and a sprinkle of sage in cooking is very unlikely to cause trouble. Concentrated sage extracts or the essential oil are where I'd be more cautious. If you take this regularly, just run it past your pharmacist or doctor first.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2D6. So far, this interaction has not been reported in humans.

Here's the wrinkle with Hycodan or Tussigon: the hydrocodone part is actually a prodrug in the sense that your liver enzyme CYP2D6 converts some of it into hydromorphone, a more potent form. Sage, in lab studies, appears to gently block that same enzyme. So in theory, less of the drug gets converted, which could mean slightly weaker pain or cough relief for some people.
I want to be honest, though: this comes only from test-tube research and has never actually shown up in real patients. Cooking with sage won't do a thing. Even concentrated sage supplements are a small concern at most. Hydrocodone still has plenty of opioid effect on its own (drowsiness, constipation, slowed breathing), so always run any new supplement past your pharmacist or doctor first.

This cough medicine combines hydrocodone (an opioid) with homatropine. The hydrocodone part is handled partly by a liver enzyme called CYP3A4. Lab studies hint that sage might slow that enzyme down. In theory, if hydrocodone clears more slowly, a bit more could build up, which could nudge up its effects like drowsiness, dizziness, constipation, or slowed breathing.
Here's the honest part: this is only seen in test-tube studies, not in real people, so it's theoretical and likely small. Cooking with sage almost certainly won't matter. Concentrated sage extracts or capsules are where I'd be more cautious. Since this medicine already causes sleepiness, check with your pharmacist or doctor before pairing it with sage supplements.

ANTICHOLINERGIC DRUGS

In vitro evidence suggests that common sage (Salvia officinalis) and Spanish sage (Salvia lavandulaefolia) can inhibit acetylcholinesterase and might increase acetylcholine levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This cough medicine pairs hydrocodone with homatropine. The part that matters here is the homatropine, which is an anticholinergic, meaning it works by quieting down a brain and body chemical called acetylcholine. Sage may push in the opposite direction. In lab studies, sage compounds seem to block the enzyme that breaks down acetylcholine, which could leave more of it around and slightly blunt the homatropine's effect.
In real life this is a theoretical concern only, based on test-tube research, not on people. Food amounts of sage in cooking are very unlikely to do anything. Concentrated sage extracts or oils are the bigger question mark. If you're using either daily, just mention it to your pharmacist so we can keep an eye on things.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory.
