Here's the simple version: hydromorphone is a strong opioid pain reliever, and one of its main effects is calming down your central nervous system. That means it can make you drowsy, slow your breathing, and lower your alertness. Some of the natural compounds in sage appear to have their own mild calming, or sedative, activity. Stack the two together and, in theory, you could feel sleepier or more foggy than usual.

Keep this in perspective, though. This concern comes only from lab and animal studies, not from real-world reports in people, so it's a theoretical caution. Sprinkling sage on your dinner is very unlikely to be an issue. Concentrated sage extracts or the essential oil are where I'd be more careful. If you take hydromorphone daily, just run your sage supplement past your pharmacist or doctor first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

What Is Hydromorphone Used For?

Brand names: Dilaudid, Hydromorph Contin, Palladone

Hydromorphone treats pain that is severe enough to need an opioid pain medicine when other treatments have not worked, are not tolerated, or would not be enough.

Some products have narrower uses. Hydromorphone extended-release tablets are for severe, persistent pain that needs daily, around-the-clock treatment for a long period. They are not for as-needed use. The high potency injection (HPF) is only for people who are already opioid tolerant (used to taking regular opioid doses).
