Levobupivacaine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Levobupivacaine (Chirocaine) is a numbing medication, the kind your doctor injects for surgery, an epidural, or a nerve block. Your liver clears part of it using an enzyme called CYP3A4. In the test tube, sage appears to slow that enzyme down a bit. In theory, that could let levobupivacaine linger longer in your system, which might mean stronger or longer-lasting effects.

Here's my honest take: this is based only on lab data, not on real people, so I wouldn't lose sleep over it. Cooking with sage is a non-issue. Since this drug is given in a controlled medical setting, just tell your anesthesia team or doctor about any concentrated sage extracts or sage oil you take, and let them guide you.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory.
