ANTIHYPERTENSIVE DRUGS
Why this applies to Treprostinil
Treprostinil falls under ANTIHYPERTENSIVE DRUGS — the group Sage interacts with.
Animal research suggests that common sage (Salvia officinalis) can cause prolonged blood pressure reduction. However, clinical research suggests that Spanish sage (Salvia lavandulaefolia) can increase blood pressure in some people with hypertension. Until more is known, use with caution.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Treprostinil (Orenitram) widens blood vessels to ease the strain on your heart and lungs, and one common effect is lowering your blood pressure. Sage is on file as something that could nudge blood pressure in either direction. In animal studies, common sage tended to lower it, while in some people Spanish sage actually raised it. So sage could either add to Treprostinil's blood-pressure-lowering effect or work against it.
I'll be honest: this is theoretical and based on lab and animal work, not solid human studies, so I wouldn't panic. Cooking with sage is very unlikely to matter; concentrated extracts or sage oil are the bigger question. If you're considering a sage supplement, please run it past your pharmacist or doctor first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.
About Treprostinil
What Is Treprostinil Used For?
Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.
Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
