Amifampridine Phosphate falls under CHOLINERGIC DRUGS — the group Syrian Rue interacts with.
Syrian rue contains harmine. Animal research suggests that intravenous administration of harmine potentiates the effects of acetylcholine or increases acetylcholine levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Firdapse (amifampridine) works by boosting nerve-to-muscle signaling, which raises acetylcholine activity to help with the muscle weakness of Lambert-Eaton syndrome. Syrian Rue contains harmine, and animal studies hint that it may also push acetylcholine levels up. So the theory here is that combining the two could stack cholinergic effects on top of each other and make Firdapse's side effects more noticeable.
In real life that might look like more tingling or pins-and-needles, stomach cramping, nausea, or in larger doses a higher chance of seizures. I want to be honest though: this is a theoretical concern based on lab and animal work, not proven in people. Syrian Rue also carries its own toxicity risks, so please check with your doctor or pharmacist before mixing them.

Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment. Talk to a pharmacist or doctor before considering it.
