Theoretically, Syrian rue might increase the risk of serotonergic adverse effects, including serotonin syndrome, when taken with serotonergic drugs.
Syrian rue contains harmaline and harmine, which have demonstrated strong, reversible inhibition of monoamine oxidase (MAO) A and weak inhibition of MAO B in animal and in vitro research. This may increase the risk for adverse effects with serotonergic drugs.

This combination product (Tussend) carries two pieces worth watching. Syrian Rue's harmala alkaloids act like a natural MAO inhibitor, which can let brain chemicals like serotonin pile up. The pseudoephedrine here is a decongestant that bumps up adrenaline-type signals, and the hydrocodone has mild serotonin activity too. Stacking these on top of an MAOI could raise the odds of a jittery, overstimulated reaction or, rarely, serotonin syndrome (agitation, racing heart, sweating, high blood pressure, shaking).

Keep in mind this warning is theoretical and comes only from lab and animal studies, so the real-world risk in people isn't well proven. Still, because the pieces line up in a worrying direction, I'd run this past your pharmacist or doctor before mixing them rather than testing it yourself.

Syrian rue contains beta-carboline alkaloids, including harmine, harmaline, harmalol, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids can inhibit CYP2D6 enzymes.

This combination product is tricky because one of its ingredients works backwards from the others. Hydrocodone is a prodrug that your liver's CYP2D6 enzyme turns into hydromorphone, the form that gives stronger pain relief. Syrian Rue may block CYP2D6, so in theory it could leave hydrocodone less able to convert, meaning you might feel less pain relief from that part.

On the flip side, the chlorpheniramine (an antihistamine) is also handled by CYP2D6, so its levels could climb, which may mean more drowsiness, dry mouth, or dizziness. There's another worry too: Syrian Rue acts like an MAO inhibitor, and mixing MAOIs with this kind of product can be risky. This is all theoretical, drawn from lab data, but please check with your pharmacist or doctor before combining them.

Here's the thing to keep in mind with this combo medicine (Tussend) and Syrian Rue. The hydrocodone part is handled in your body partly by an enzyme called CYP3A4. Lab studies hint that Syrian Rue's compounds can slow that enzyme down, so in theory hydrocodone could hang around a bit longer and feel stronger. That might mean more drowsiness, dizziness, constipation, or slowed breathing.

There's a separate worry too: Syrian Rue acts like an MAO inhibitor, and mixing MAO inhibitors with cough and cold products that contain pseudoephedrine (a decongestant) can spike blood pressure. This is all theoretical and based on lab work, not solid human data, but please don't combine these without checking with your pharmacist or doctor first.

Tussend is a combination product, and the piece that matters here is the chlorpheniramine, an antihistamine that works partly by blocking acetylcholine (an anticholinergic effect). Syrian Rue contains harmine, which in animal studies seems to nudge acetylcholine levels up. In plain terms, the supplement could theoretically push in the opposite direction from chlorpheniramine, so the antihistamine might work a little less well for things like a runny nose or sneezing. This is theory based on lab and animal data only, not proven in people.

Honestly, the bigger concern is that Syrian Rue acts like an MAO inhibitor, and combining MAOIs with decongestants like pseudoephedrine or with opioids like hydrocodone can be genuinely risky. Please check with your pharmacist or doctor before mixing these.
