Syrian rue contains harmaline and harmine, which have demonstrated strong, reversible inhibition of monoamine oxidase (MAO) A and weak inhibition of MAO B in animal and in vitro research. This may increase the risk for adverse effects with serotonergic drugs.

This combo cough and cold product pairs an antihistamine (chlorpheniramine) with a cough suppressant (dextromethorphan). The one we worry about here is dextromethorphan, which gently raises serotonin in the brain. Syrian Rue acts like an MAO inhibitor, meaning it slows the breakdown of serotonin. Stack the two together and serotonin can climb higher than it should.

In theory, that could trigger serotonin syndrome, with symptoms like agitation, a racing heart, sweating, shaking, muscle twitching, or confusion. Keep in mind this warning comes from lab and animal research, not solid human studies, so it is more caution than proof. Still, since Syrian Rue carries its own toxicity risks, I would not combine these without first checking with your pharmacist or doctor.

This combo cough and cold product contains dextromethorphan, which your body clears mainly through a liver enzyme called CYP2D6. Lab studies suggest Syrian Rue can block that enzyme, so in theory the dextromethorphan could build up and hit harder. That might mean more drowsiness, dizziness, confusion, or feeling lightheaded.

There's an extra worry here: dextromethorphan raises serotonin, and Syrian Rue acts like an MAO inhibitor. Putting those together can risk too much serotonin, which is potentially dangerous. The chlorpheniramine adds its own sedation and dry-mouth effects on top. This is theoretical and based on lab data, not strong human studies, but given the MAOI overlap I'd play it safe. Please check with your pharmacist or doctor before mixing these.

This cough and cold combo pairs chlorpheniramine (an antihistamine) with dextromethorphan (a cough suppressant). Both are partly broken down by a liver enzyme called CYP3A4, and dextromethorphan also runs through CYP2D6. Lab studies suggest the harmala alkaloids in Syrian Rue can block CYP3A4, so in theory these medicines could clear more slowly and build up. That might mean more drowsiness, dizziness, dry mouth, or feeling foggy than usual.

The bigger concern is that Syrian Rue acts like an MAO inhibitor, and dextromethorphan plus MAOIs can rarely cause a dangerous reaction called serotonin syndrome. The CYP3A4 part is only theoretical, but I'd still run this combination past your pharmacist or doctor first.

This combo product pairs chlorpheniramine, an antihistamine with drying (anticholinergic) effects, and dextromethorphan, a cough suppressant. The concern on file is mostly with the chlorpheniramine piece. Chlorpheniramine works partly by blocking the action of acetylcholine, a chemical messenger in the body. Syrian Rue's harmala alkaloids may push acetylcholine levels the other way, so in theory they could blunt some of chlorpheniramine's effect.

Keep in mind this is theoretical, based on animal lab work, not real human cases. There's a bigger worry, though: Syrian Rue acts like an MAO inhibitor, and dextromethorphan plus MAOIs can be a dangerous mix (think serotonin syndrome, with agitation, fever, and racing heart). I'd steer clear of combining these and check with your pharmacist first.
