Elbasvir, Grazoprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Syrian Rue interacts with.

Theoretically, Syrian rue might increase the levels and clinical effects of drugs metabolized by CYP3A4.
Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.

Here's the basic idea with Zepatier (elbasvir and grazoprevir) and Syrian Rue. Your body uses an enzyme called CYP3A4 to help clear this hepatitis C medicine. Lab studies suggest the harmala alkaloids in Syrian Rue can slow that enzyme down. If that happened in people, the medication could build up a bit higher than expected, which might mean more side effects like tiredness, headache, nausea, or stress on the liver (and grazoprevir levels in particular are sensitive to this kind of blocking).

I want to be honest: this concern is theoretical, based only on test-tube research, not real patient cases. Still, since Syrian Rue carries its own toxicity risks, please check with your pharmacist or doctor before combining the two.

What Is Elbasvir and Grazoprevir Used For?

Brand name: Zepatier

Zepatier treats chronic hepatitis C virus (HCV) genotype 1 or genotype 4 infection in adults and children 12 years of age and older (or weighing at least 30 kg). It works for people who have never been treated before as well as those who have tried certain other treatments.

Depending on the specific genotype and treatment history, Zepatier may be prescribed on its own or combined with another antiviral medicine called ribavirin. Your prescriber will determine the right combination and length of treatment for your situation.
