Macitentan, Tadalafil falls under HEPATOTOXIC DRUGS — the group Syrian Rue interacts with.

Theoretically, high doses of Syrian rue might increase the risk of liver damage when taken with hepatotoxic drugs.
Cases of hepatotoxicity and hepatic failure have been reported in patients who ingested a large quantity (up to 100 grams) of Syrian rue seeds.

Here's the concern with Opsynvi, which combines macitentan and tadalafil. Macitentan is one of those medicines that can occasionally stress the liver, so your doctor may already check your liver blood tests while you take it. Syrian Rue enters the picture because reports show that swallowing large amounts of the seeds (people have taken close to 100 grams) has caused liver injury on its own.

Put the two together and, at least in theory, you could add strain on top of strain. This isn't proven for this exact combo, it's a reasoned worry based on how each affects the liver. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your pharmacist or doctor before using Syrian Rue.

Theoretically, Syrian rue might increase the levels and clinical effects of drugs metabolized by CYP3A4.
Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.

Opsynvi is a two-in-one pill that combines macitentan (which relaxes blood vessels in the lungs) and tadalafil (a Cialis-type medicine that also opens up vessels). Both ingredients are cleared by a liver enzyme called CYP3A4. In lab studies, the harmala alkaloids in Syrian rue appear to slow that same enzyme down. In theory, that could let these drugs build up in your body and hit harder than intended.

What that might feel like: more dizziness, low blood pressure, flushing, headache, or a stuffy nose. Keep in mind this is a theoretical concern based only on test-tube data, not real patient reports. Still, since Syrian rue is risky on its own, please check with your pharmacist or doctor before combining them.

There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment.
