Here's the concern with pairing Syrian Rue and methotrexate: both can be tough on your liver. Methotrexate is well known for raising liver enzymes and, over time, causing liver damage, which is why your doctor checks bloodwork regularly while you take it. Syrian Rue seeds, especially in large or concentrated amounts, have been linked to liver injury too. Stacking two liver stressors together could add up.

I'll be honest, this is a theoretical caution based on how these substances behave, not on solid human studies, and the reported liver problems came from people eating huge amounts of the seeds (up to 100 grams). Still, since your liver already works hard on methotrexate, I'd skip Syrian Rue and check with your doctor or pharmacist first.

What is Syrian Rue?

Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment. What Is Methotrexate Used For?

Brand names: Rheumatrex, Otrexup, Rasuvo, Reditrex

Methotrexate treats a wide range of serious conditions. In cancer care, it is used for acute lymphoblastic leukemia, non-Hodgkin lymphoma, osteosarcoma, breast cancer, head and neck cancer, and gestational trophoblastic neoplasia (a rare tumor related to pregnancy). It is also used to prevent and treat meningeal leukemia (cancer affecting the lining of the brain and spinal cord).

Outside of cancer, methotrexate is prescribed for adults with rheumatoid arthritis (RA), children with polyarticular juvenile idiopathic arthritis (a form of childhood arthritis), and adults with severe psoriasis that hasn't responded well to other treatments.
