Why this applies to Darolutamide

Darolutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.

In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4). This suggests that tangeretin may stimulate CYP3A4 activity. However, in humans, drinking tangerine juice 200 mL slightly delayed the absorption, but did not affect the metabolism, of midazolam, a CYP3A4 substrate. Theoretically, tangerine juice might increase CYP3A4 activity and decrease levels of drugs metabolized by this enzyme. However, this effect is unlikely.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.

Here's the short version: your prostate cancer medication darolutamide (Nubeqa) is broken down by a liver enzyme called CYP3A4. There's some lab work suggesting a compound in tangerine (tangeretin) might speed that enzyme up a little. If that happened in real life, your body could clear darolutamide a bit faster, which in theory means slightly less of it working against the cancer.

The good news is that this is mostly a test-tube finding. When people actually drank tangerine juice, it didn't change how a similar drug was processed. So enjoying tangerines as food is very unlikely to cause a problem. I still wouldn't load up on concentrated tangerine peel or supplement extracts without checking. Since darolutamide is important cancer therapy, run any tangerine supplements past your oncologist or pharmacist first.

What Is Darolutamide Used For?

Brand name: Nubeqa

Nubeqa (darolutamide) tablets are approved for adult men with two forms of advanced prostate cancer. The first is non-metastatic castration-resistant prostate cancer (nmCRPC) — cancer that keeps growing despite hormone-lowering therapy and has not yet spread to other parts of the body. The second is metastatic castration-sensitive prostate cancer (mCSPC) — cancer that has spread but still responds to hormone-lowering treatment.

For mCSPC, Nubeqa can be used alone or in combination with docetaxel (a chemotherapy medicine). In all cases, patients also receive a gonadotropin-releasing hormone (GnRH) agonist or antagonist — a medicine that lowers testosterone — or have already had surgery to remove the testicles.
