Why this applies to Hydrocortisone

Hydrocortisone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.

In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4). This suggests that tangeretin may stimulate CYP3A4 activity. However, in humans, drinking tangerine juice 200 mL slightly delayed the absorption, but did not affect the metabolism, of midazolam, a CYP3A4 substrate. Theoretically, tangerine juice might increase CYP3A4 activity and decrease levels of drugs metabolized by this enzyme. However, this effect is unlikely.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.

Here's the gist for tangerine and hydrocortisone. Your body clears hydrocortisone using a liver enzyme called CYP3A4. A compound in tangerine (tangeretin) has been shown in the lab to slightly speed up that enzyme. In theory, that could clear hydrocortisone a bit faster and make it work a touch less, but this is mostly test-tube data.

When researchers actually tested tangerine juice in people, it did not meaningfully change how a similar drug was processed. So in real life, eating tangerines or drinking a normal amount of juice is very unlikely to weaken your hydrocortisone or worsen the condition you're treating. If you take large concentrated tangerine peel supplements, just mention it to your pharmacist to be safe.

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
