Why this applies to Quinidine

Quinidine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.

In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4). This suggests that tangeretin may stimulate CYP3A4 activity. However, in humans, drinking tangerine juice 200 mL slightly delayed the absorption, but did not affect the metabolism, of midazolam, a CYP3A4 substrate. Theoretically, tangerine juice might increase CYP3A4 activity and decrease levels of drugs metabolized by this enzyme. However, this effect is unlikely.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.

Here's the gist: your body clears quinidine partly through a liver enzyme called CYP3A4. Lab studies hint that a compound in tangerine (tangeretin) might speed that enzyme up a little. If that happened, your body could break down quinidine slightly faster, which in theory could nudge its blood levels down and make it a touch less effective at steadying your heart rhythm.

I want to be honest with you, though: in actual human testing, tangerine juice did not meaningfully change how this enzyme worked. So this is mostly a theoretical concern, and the real-world risk is considered low. Since quinidine is a drug where steady levels matter, I'd still mention your tangerine habit to your pharmacist or doctor, but you don't need to panic over eating one.

About Quinidine

What Is Quinidine Used For?

Brand names: Quinaglute Duratabs, Quinaglute, Duraquin, Cardioquin, Cin-Quin, Quinora, Lin-Qin, Quinova, Quinidex Extentabs

Quinidine is used to help restore a normal heart rhythm in people with atrial fibrillation or atrial flutter (irregular, rapid heart rhythms originating in the upper chambers). It is also used on an ongoing basis to reduce how often those episodes come back, in patients whose episodes are frequent and poorly tolerated — but only after other options have been tried first.

Quinidine sulfate oral tablets and quinidine gluconate extended-release oral tablets are both approved for these heart-rhythm uses. Quinidine sulfate tablets are additionally used as part of an approved treatment regimen for life-threatening Plasmodium falciparum malaria, in combination with quinidine gluconate injection (the central component of that regimen). Both formulations may also be used to suppress recurrent, documented life-threatening ventricular arrhythmias (dangerous rapid rhythms in the lower chambers of the heart), such as sustained ventricular tachycardia.
