Tangerine has a minor interaction with Tramadol. In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4).

Why this applies to Tramadol
Tramadol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the interesting twist with tramadol: it's actually a prodrug, meaning your body has to convert it into its active pain-relieving form. A compound in tangerine called tangeretin showed, in lab studies, a mild ability to speed up the CYP3A4 enzyme. In theory, faster CYP3A4 activity could shift how tramadol is processed and slightly lower the amount circulating, which might mean a bit less pain relief.
But honestly, I wouldn't lose any sleep over this one. When researchers tested actual tangerine juice in people, it didn't meaningfully change drug metabolism. Eating tangerines as food is very unlikely to affect your pain control. If you're using concentrated tangerine peel extracts or notice your tramadol working differently, just check in with your pharmacist or doctor.
