Why this applies to Oxybutynin

Oxybutynin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.

In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4). This suggests that tangeretin may stimulate CYP3A4 activity. However, in humans, drinking tangerine juice 200 mL slightly delayed the absorption, but did not affect the metabolism, of midazolam, a CYP3A4 substrate. Theoretically, tangerine juice might increase CYP3A4 activity and decrease levels of drugs metabolized by this enzyme. However, this effect is unlikely.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.

Here's the gist: your body uses an enzyme called CYP3A4 to break down oxybutynin (Ditropan and the patch versions, Oxytrol). In a lab dish, one tangerine compound (tangeretin) sped that enzyme up a bit, which in theory could clear oxybutynin a little faster and make it work less well. If that happened, you might notice your overactive bladder symptoms, like urgency or frequent trips to the bathroom, creeping back.

The good news is that in actual human testing, tangerine juice did not meaningfully change how a similar drug was processed. So this is mostly theoretical, the effect is rated minor and unlikely, and eating a tangerine or two as food shouldn't be a problem. If you load up on concentrated tangerine peel supplements, just mention it to your pharmacist or doctor to be safe.

About Oxybutynin

What Is Oxybutynin Used For?

Brand names: Ditropan, Ditropan XL, Oxybutynin, Oxytrol, Anturol, Gelnique

Oxybutynin treats overactive bladder — including the sudden urge to urinate, leaking before you can reach the bathroom (urge incontinence), and going too frequently. Extended-release tablets (Oxybutynin Chloride) and immediate-release tablets and oral solution (Oxybutynin Chloride) are used for these symptoms. Oxytrol is approved for overactive bladder in men; Oxytrol for Women is approved for overactive bladder in women aged 18 and older.

Extended-release tablets (Oxybutynin Chloride) are also approved for children 6 years and older who have bladder overactivity caused by a neurological condition, such as spina bifida. Immediate-release tablets and oral solution (Oxybutynin Chloride) are indicated for bladder instability symptoms — including urgency, frequency, urinary leakage, and painful urination — in patients with neurogenic bladder conditions.
