Why this applies to Carbamazepine

Carbamazepine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.

In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4). This suggests that tangeretin may stimulate CYP3A4 activity. However, in humans, drinking tangerine juice 200 mL slightly delayed the absorption, but did not affect the metabolism, of midazolam, a CYP3A4 substrate. Theoretically, tangerine juice might increase CYP3A4 activity and decrease levels of drugs metabolized by this enzyme. However, this effect is unlikely.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.

Here's the short version: tangerine contains a compound called tangeretin that, in a lab dish, sped up the CYP3A4 enzyme, which is one of the same enzymes that helps clear carbamazepine from your body. In theory, revving up that enzyme could lower your carbamazepine levels and make it work less well, which for a seizure or trigeminal neuralgia medicine could mean breakthrough seizures or returning nerve pain.

The reassuring part is that when this was actually tested in people (using tangerine juice), it did not change how a CYP3A4 drug was broken down. So this concern is mostly theoretical, and normal amounts of tangerine as food are very unlikely to matter. Still, carbamazepine is a sensitive drug, so check with your pharmacist or doctor before using concentrated tangerine peel or oil supplements.

About Carbamazepine

What Is Carbamazepine Used For?

Brand names: Tegretol, Tegretol XR, Equetro, Carbatrol, Carnexiv

Carbamazepine is used to treat certain types of seizures — including partial seizures, generalized tonic-clonic (grand mal) seizures, and mixed seizure patterns. It works best for those specific seizure types; it does not appear to control absence (petit mal) seizures.

It is also prescribed to relieve the intense facial pain of trigeminal neuralgia — a nerve condition that causes sudden, severe pain along the face. It is not a general painkiller and shouldn't be used for ordinary aches and pains.
