Dextromethorphan HBr, Phenylephrine HCl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Valerian interacts with.

Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP2D6.
Although some in vitro evidence suggests that valerian affects CYP2D6, clinical pharmacokinetic (PK) studies show that valerian is unlikely to affect the CYP2D6 enzyme. In one PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days did not affect the metabolism of dextromethorphan, a CYP2D6 substrate. In another PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of debrisoquine, an accepted CYP2D6 probe-substrate.

Good news here: when it comes to valerian and the dextromethorphan in your Vicks Formula 44D, the worry is mostly theoretical. Early lab-dish studies hinted that valerian might block CYP2D6, the liver enzyme that helps clear dextromethorphan. But actual human studies told a different story. In one trial, people took 1000 mg of valerian nightly for two weeks and it did not meaningfully change how their bodies handled dextromethorphan.

So you're unlikely to see a real buildup or extra drowsiness from this combo on a metabolism level. That said, both valerian and dextromethorphan can make you a bit sleepy on their own, so together they might add up to more grogginess. The phenylephrine isn't a concern here. If you're unsure, just run it by your pharmacist.

Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP3A4.
Although some in vitro evidence suggests that valerian extract might inhibit or induce CYP3A4, clinical pharmacokinetic (PK) studies show that valerian does not have a clinically significant effect on the CYP3A4 enzyme. In one PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of midazolam, an accepted CYP3A4 probe-substrate. In another PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days modestly increases levels of alprazolam, a CYP3A4 substrate, suggesting mild inhibition of CYP3A4. However, this mild inhibition is unlikely to be clinically relevant.

Good news here: when folks ask about pairing valerian with a cough and congestion product like Vicks Formula 44D (which contains dextromethorphan and phenylephrine), the metabolism worry is pretty small. Some test-tube studies hinted valerian might nudge the CYP3A4 enzyme that helps clear certain drugs, but real human studies show any effect is mild at most and not clinically meaningful. So I wouldn't expect valerian to meaningfully change how your body handles these ingredients.

The thing I'd actually keep an eye on is the sedation. Valerian can make you drowsy, and dextromethorphan can too, so together you might feel sleepier or foggier. If you also take prescription sedatives or antidepressants, check with your pharmacist first to be safe.
