CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
Why this applies to Paroxetine
Paroxetine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Valerian interacts with.
Although some in vitro evidence suggests that valerian affects CYP2D6, clinical pharmacokinetic (PK) studies show that valerian is unlikely to affect the CYP2D6 enzyme. In one PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days did not affect the metabolism of dextromethorphan, a CYP2D6 substrate. In another PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of debrisoquine, an accepted CYP2D6 probe-substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Good news here: the concern with valerian and paroxetine is mostly theoretical, and the real-world studies are reassuring. Paroxetine is cleared in part by a liver enzyme called CYP2D6. Some lab tests hinted that valerian might block that enzyme, which could in theory let paroxetine build up. But actual human studies using valerian didn't show any meaningful effect on CYP2D6, so this interaction is rated minor and unlikely.
Paroxetine is an active drug (not a prodrug), so it doesn't get switched on by this enzyme. That said, both can cause drowsiness, so combining them might leave you feeling extra sleepy or foggy. If you're considering valerian for sleep, it's worth a quick chat with your pharmacist or doctor first.

Paroxetine tablets and oral suspension (including Paxil) are used in adults for major depressive disorder, obsessive compulsive disorder (OCD), panic disorder, social anxiety disorder, generalized anxiety disorder, and posttraumatic stress disorder (PTSD).
Paroxetine extended-release tablets (including Paxil CR) are used in adults for major depressive disorder, panic disorder, social anxiety disorder, and premenstrual dysphoric disorder (PMDD, a severe form of premenstrual mood symptoms). Brisdelle and paroxetine capsules are only for moderate to severe hot flashes and night sweats (vasomotor symptoms) from menopause. They are not for any psychiatric condition.
