Why this applies to Nortriptyline

Nortriptyline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Valerian interacts with.

Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP2D6.
Although some in vitro evidence suggests that valerian affects CYP2D6, clinical pharmacokinetic (PK) studies show that valerian is unlikely to affect the CYP2D6 enzyme. In one PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days did not affect the metabolism of dextromethorphan, a CYP2D6 substrate. In another PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of debrisoquine, an accepted CYP2D6 probe-substrate.

Good news here: the lab worry about Valerian slowing down the CYP2D6 enzyme (one of the main pathways your body uses to clear Nortriptyline) hasn't really panned out in people. Human studies measuring this enzyme found Valerian didn't meaningfully change how it works, so Valerian is unlikely to cause your Nortriptyline to build up in your blood.

The thing I'd actually keep an eye on is a different, more practical overlap: both can make you drowsy. Nortriptyline already tends to cause sedation, and Valerian is taken to help with sleep, so together you may feel extra sleepy or groggy, especially during the day. That's usually mild, but check with your pharmacist or doctor before combining them.

Nortriptyline is approved to relieve symptoms of depression. This applies to nortriptyline hydrochloride capsules, nortriptyline hydrochloride oral solution, and Pamelor capsules — all used for the same core indication.

The label notes that depression arising from within the body (endogenous depression) may respond better than other types of depression, though all of these products are indicated for the broader relief of depressive symptoms.
