Why this applies to Timolol

Timolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Valerian interacts with.

Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP2D6.
Although some in vitro evidence suggests that valerian affects CYP2D6, clinical pharmacokinetic (PK) studies show that valerian is unlikely to affect the CYP2D6 enzyme. In one PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days did not affect the metabolism of dextromethorphan, a CYP2D6 substrate. In another PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of debrisoquine, an accepted CYP2D6 probe-substrate.

Here's the good news on pairing valerian with timolol: while early test-tube studies hinted that valerian might tinker with CYP2D6 (the liver enzyme that helps clear timolol), the actual human studies just don't back that up. When researchers gave people valerian for two to four weeks, it didn't meaningfully change how the body processed CYP2D6 drugs. So in real life, valerian is unlikely to cause timolol to build up or wear off faster.

The practical thing to keep in mind is overlap of effects, not metabolism. Valerian can make you drowsy, and timolol can already leave some folks feeling tired, lightheaded, or sluggish, so together you might notice more of that. If you're starting valerian for sleep, it's worth a quick word with your pharmacist or doctor first.

What Is Timolol Used For?

Brand names: Blocadren, Timol, Timoptic

Timolol is available in ophthalmic (eye) forms and as an oral tablet, and each form has its own approved uses. Ophthalmic formulations — including Betimol, Istalol, Timolol GFS, Timolol hemihydrate, Timolol Maleate eye drops, and Timolol Maleate Ophthalmic Gel Forming Solution — are used to lower elevated intraocular pressure (pressure inside the eye) in people with ocular hypertension or open-angle glaucoma.

Timolol Maleate tablets are used orally for three separate conditions: treating high blood pressure (alone or combined with other blood pressure medicines), reducing the risk of death and repeat heart attack in people who have survived a heart attack and are clinically stable, and preventing migraine headaches.
