CNS DEPRESSANTS

Acetaminophen, Caffeine, Dihydrocodeine falls under CNS DEPRESSANTS — the group Valerian interacts with.
Theoretically, concomitant use of valerian and drugs with sedative and anesthetic properties may cause additive therapeutic and adverse effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product contains dihydrocodeine, an opioid pain reliever that already causes drowsiness, slowed breathing, and that foggy, sleepy feeling. Valerian is used as a calming sleep aid and may have its own mild sedative effect on the brain. Put them together and those calming effects can stack, so you might feel more drowsy, dizzy, or out of it than you expect, with a small added risk of breathing being slowed too much.
To be fair, this concern comes mostly from lab and animal research, not strong human studies, so it's more theoretical than proven. The caffeine in this product won't reliably cancel it out either. Don't stop anything on your own, but do check with your pharmacist or doctor before pairing valerian with this medicine.

GLUCURONIDATED DRUGS

In vitro research shows that methanolic valerian extract and valerenic acid might competitively inhibit UDP-glucuronosyltransferase (UGT) 1A1 (UGT1A1) and UGT2B7.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. This combo pill contains acetaminophen, caffeine, and dihydrocodeine for pain. A small piece of how your body clears acetaminophen relies on a process called glucuronidation. In lab and animal studies, valerian seems to slow that process a little, which in theory could let acetaminophen build up slightly. In real life, this is a weak, test-tube-level finding, so I wouldn't lose sleep over it.
The bigger thing on my mind is overlap of effects. Dihydrocodeine is an opioid, and valerian is a calming, sleep-promoting herb, so together they could add up to more drowsiness, dizziness, or grogginess. If you take this medicine and want to try valerian, just run it by your pharmacist or doctor first.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
3 of 4Acetaminophen, Caffeine, Dihydrocodeine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
Although some in vitro evidence suggests that valerian affects CYP2D6, clinical pharmacokinetic (PK) studies show that valerian is unlikely to affect the CYP2D6 enzyme. In one PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days did not affect the metabolism of dextromethorphan, a CYP2D6 substrate. In another PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of debrisoquine, an accepted CYP2D6 probe-substrate.

Good news here: the worry on paper is bigger than the worry in real life. Dihydrocodeine is a prodrug, meaning your liver enzyme CYP2D6 converts it into the stronger active painkiller that does most of the work. Early lab tests hinted that valerian might block that enzyme, which in theory could leave you with weaker pain relief. But actual studies in people did not find this. When folks took valerian for days or weeks, it did not meaningfully change how CYP2D6 handled its test drugs.
So for this combo, valerian is unlikely to weaken your pain relief or change the acetaminophen or caffeine. The bigger thing to mention to your pharmacist is that both valerian and dihydrocodeine can make you drowsy, so go easy and avoid driving until you know how you feel.

Although some in vitro evidence suggests that valerian extract might inhibit or induce CYP3A4, clinical pharmacokinetic (PK) studies show that valerian does not have a clinically significant effect on the CYP3A4 enzyme. In one PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of midazolam, an accepted CYP3A4 probe-substrate. In another PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days modestly increases levels of alprazolam, a CYP3A4 substrate, suggesting mild inhibition of CYP3A4. However, this mild inhibition is unlikely to be clinically relevant.

Good news here: the main concern people raise about valerian and this combo pill (which contains the opioid dihydrocodeine plus acetaminophen and caffeine) is whether valerian changes how your body breaks the medicine down through an enzyme called CYP3A4. Real-world human studies suggest valerian's effect on that enzyme is mild at most and not enough to matter clinically, so it's unlikely to make your pain reliever build up or wear off.
The thing I'd actually keep an eye on is overlapping sedation. Valerian is used to calm and relax you, and dihydrocodeine can already cause drowsiness, dizziness, and slowed breathing. Together they may make you sleepier or foggier than expected. Please check with your pharmacist or doctor before pairing them, especially if you take other calming medicines.
