Why this applies to Metoprolol

Metoprolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Valerian interacts with.

Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP2D6.
Although some in vitro evidence suggests that valerian affects CYP2D6, clinical pharmacokinetic (PK) studies show that valerian is unlikely to affect the CYP2D6 enzyme. In one PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days did not affect the metabolism of dextromethorphan, a CYP2D6 substrate. In another PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of debrisoquine, an accepted CYP2D6 probe-substrate.

Good news here: this pairing looks pretty low-risk. Your body uses an enzyme called CYP2D6 to break down metoprolol, and lab tests once hinted that valerian might block that enzyme. If it did, metoprolol could build up and slow your heart rate or drop your blood pressure too much. But when researchers actually tested valerian in people, it did not meaningfully change how this enzyme works, so a real-world problem is unlikely.

One thing worth keeping in mind: both valerian and metoprolol can leave you feeling a little tired or drowsy, so together you might notice that more. Don't start or stop anything on your own. Just give your pharmacist or doctor a heads-up so they can keep an eye on how you feel.

About Metoprolol

What Is Metoprolol Used For?

Brand names: Lopressor, Toprol XL

Metoprolol is used to treat several heart and blood pressure conditions. Metoprolol succinate extended-release tablets (including Toprol-XL) are approved for high blood pressure, angina pectoris (chest pain from the heart), and heart failure. Metoprolol tartrate tablets (including Lopressor) are approved for high blood pressure, angina, and — when used alongside intravenous metoprolol — to reduce the risk of cardiovascular death after a definite or suspected heart attack.

Metoprolol tartrate injection (intravenous form) is used in hemodynamically stable patients with a definite or suspected acute heart attack to reduce cardiovascular death, and is followed by oral metoprolol therapy. Lowering blood pressure with metoprolol reduces the risk of strokes and heart attacks.
