Here's the thing with Phrenilin #3: it already has two ingredients that calm your brain down. The butalbital is a barbiturate, and the codeine is an opioid, so both can leave you drowsy, foggy, or slow to react. Valerian works in a similar relaxing direction, so stacking them together may pile on those sedating effects.

In real life that could mean feeling extra sleepy, dizzy, or lightheaded, with slower breathing and slower reflexes (think driving or stairs). To be fair, this combo concern comes mainly from lab and animal research, not solid human studies, so it's more of a theoretical caution than a proven problem. Still, before pairing them, it's worth a quick chat with your pharmacist or doctor.

Here's the gist with this combo pill (which packs acetaminophen, the barbiturate butalbital, and codeine). The concern is that valerian might gently slow down a liver process called glucuronidation, one of the routes your body uses to clear these ingredients. If that pathway runs a little slower, levels could creep up slightly, possibly adding to drowsiness, lightheadedness, foggy thinking, or slowed breathing. On top of that, valerian itself is calming, so stacking it with a sedative like butalbital and an opioid like codeine can simply pile on the sleepiness.

Now, the honest part: this interaction comes only from lab and test-tube studies, and the effect looks weak. It may not amount to much in real life. Still, since both products can make you sleepy, please run it by your pharmacist or doctor before combining them.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Acetaminophen, Butalbital, Codeine Phosphate also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP2D6.
Although some in vitro evidence suggests that valerian affects CYP2D6, clinical pharmacokinetic (PK) studies show that valerian is unlikely to affect the CYP2D6 enzyme. In one PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days did not affect the metabolism of dextromethorphan, a CYP2D6 substrate. In another PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of debrisoquine, an accepted CYP2D6 probe-substrate.

Here's the good news on the lab-test side of things. Your headache medicine contains codeine, which is a prodrug. That means your body has to switch it on using a liver enzyme called CYP2D6 before it can ease pain. Early test-tube studies hinted valerian might tinker with that enzyme, but actual human studies (using valerian for two to four weeks) showed no real effect on it. So valerian is unlikely to weaken or boost how well the codeine works.

The bigger thing to keep in mind is overlap. Both valerian and the butalbital plus codeine in this combo can make you sleepy and slowed down. Together they may leave you extra drowsy, foggy, or unsteady. Please check with your pharmacist or doctor before pairing them, and don't drive until you know how you feel.
