Why this applies to Risperidone

Risperidone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Valerian interacts with.

Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP2D6.
Although some in vitro evidence suggests that valerian affects CYP2D6, clinical pharmacokinetic (PK) studies show that valerian is unlikely to affect the CYP2D6 enzyme. In one PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days did not affect the metabolism of dextromethorphan, a CYP2D6 substrate. In another PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of debrisoquine, an accepted CYP2D6 probe-substrate.

Good news here: when people worry about combining valerian with risperidone, the concern is usually that valerian might block CYP2D6, the liver enzyme that helps process risperidone. In theory, blocking that enzyme could let risperidone build up and bring on more drowsiness, dizziness, or lightheadedness. But the actual studies in people don't back that up. Valerian, even taken nightly for a couple of weeks, did not meaningfully change how this enzyme works.

So this is rated a minor and unlikely interaction. The bigger thing to keep in mind is that both can make you sleepy, so the combined calming effect may leave you extra groggy. Check with your pharmacist or doctor before adding valerian.

Risperidone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Valerian does not seem to have a clinically relevant effect on levels of drugs metabolized by CYP3A4.
Although some in vitro evidence suggests that valerian extract might inhibit or induce CYP3A4, clinical pharmacokinetic (PK) studies show that valerian does not have a clinically significant effect on the CYP3A4 enzyme. In one PK study, taking valerian 125 mg three times daily for 28 days did not affect metabolism of midazolam, an accepted CYP3A4 probe-substrate. In another PK study, taking valerian 1000 mg (providing about 11 mg valerenic acid) nightly for 14 days modestly increases levels of alprazolam, a CYP3A4 substrate, suggesting mild inhibition of CYP3A4. However, this mild inhibition is unlikely to be clinically relevant.

Here's the short version: your body partly clears risperidone using a liver enzyme called CYP3A4, and valerian was once suspected of slowing that enzyme down. But the actual human studies are reassuring. When researchers tested valerian against real CYP3A4 medications, it didn't meaningfully change their levels. So valerian is very unlikely to cause risperidone to build up in your system.

That said, I'd still keep one thing on your radar. Risperidone can make you drowsy on its own, and valerian is taken precisely to calm you and help you sleep. Put together, you could feel extra sleepy, foggy, or lightheaded. It's a good idea to run this combo by your pharmacist or doctor before starting valerian, especially if you drive or feel groggy.

What Is Risperidone Used For?

Brand names: Risperdal, Perseris

Risperidone is prescribed for three main conditions: schizophrenia (a disorder affecting how a person thinks, feels, and perceives reality), acute manic or mixed episodes in Bipolar I Disorder, and irritability associated with autistic disorder — including aggression, self-injury, temper tantrums, and rapid mood swings.

For bipolar mania, it can be used alone or alongside lithium or valproate. It is approved for use in both adults and certain age groups of children and adolescents, depending on the condition being treated.
