Selpercatinib falls under HEPATOTOXIC DRUGS — the group Vitamin A interacts with.

Theoretically, taking high doses of vitamin A in combination with other potentially hepatotoxic drugs might increase the risk of liver disease.
The tolerable upper intake level (UL) is the highest level of intake that is likely to pose no risk of adverse effects. Doses of vitamin A above the UL can cause hepatotoxicity, ranging from elevated liver enzymes to liver failure.

Here's the thing to know about pairing vitamin A with selpercatinib (Retevmo): this isn't about one changing the other's blood levels. Instead, both can be tough on your liver. Selpercatinib is well known for raising liver enzymes (your cancer team checks these regularly), and very high doses of vitamin A can also stress the liver. Stacked together, the worry is a bit more strain on that organ.

I'll be honest that this caution is mostly theoretical and based on expert opinion, not strong studies. Normal food amounts and a regular multivitamin are very unlikely to be a problem. The real concern is large, concentrated vitamin A supplements above the upper limit. Please don't change anything on your own. Just run any vitamin A supplement past your oncologist or pharmacist first.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
