American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with this pairing. Selpercatinib (Retevmo) is a cancer medicine that, on its own, can stretch out the heart's electrical cycle, something doctors call QT prolongation on an ECG. American Hellebore (Veratrum viride) carries its own heart risks, including this same kind of QT effect, along with dangerous drops in heart rate and blood pressure. Stacking the two could add up and raise the chance of a serious, irregular heartbeat called torsades de pointes.

I want to be honest about the evidence here, which is fairly limited for hellebore, but this plant is genuinely poisonous even in small amounts, so I wouldn't take any chances. Please don't use it alongside your selpercatinib without first talking to your oncologist or pharmacist.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
