Why this applies to Alfentanil

Alfentanil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.

Theoretically, vitamin E might induce metabolism of CYP3A4, possibly reducing the levels CYP3A4 substrates.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.

Alfentanil is a powerful, fast-acting opioid painkiller given during anesthesia and procedures, and your liver clears it using an enzyme called CYP3A4. The thought here is that high-dose vitamin E may gently speed up that enzyme, which could clear alfentanil a little faster and make it slightly less effective. So in theory you might get a bit less pain relief or a shorter effect than expected.

I want to be honest though, this is only a theoretical concern based on how the two work in the lab, not real cases of problems. Everyday food amounts of vitamin E almost certainly won't matter. Since alfentanil is given in a controlled hospital setting, just mention any vitamin E supplements to your anesthesia team or pharmacist so they're aware.
