Ketoconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.

Theoretically, vitamin E might induce metabolism of CYP3A4, possibly reducing the levels CYP3A4 substrates.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.

Here's the idea behind this pairing. Vitamin E may gently nudge your body to make more of a liver enzyme called CYP3A4, and your body uses that same enzyme to break down ketoconazole. If that enzyme speeds up, ketoconazole could clear a bit faster and its blood levels might dip, which in theory could make it work less well against your fungal infection.

I want to keep this in perspective, though. This concern is theoretical, based on how the chemistry could play out, not on real patient problems. Everyday food amounts of vitamin E are very unlikely to matter. High-dose supplements are the part worth mentioning. Before adding a vitamin E supplement, just run it by your pharmacist or doctor so they can keep your treatment on track.

What Is Ketoconazole Used For?

Brand name: Nizoral

Ketoconazole 2% shampoo and foam are used to treat tinea versicolor (also called pityriasis versicolor) — a fungal infection caused by Pityrosporum orbiculare (also known as Malassezia furfur), a yeast that naturally lives on the skin.

This infection creates light or dark patches, most often on the trunk, neck, arms, and upper thighs. Even after the fungus is cleared, those skin patches may take months to return to their normal color — that's normal, and depends on your skin type and sun exposure. Tinea versicolor can also come back, since the fungus lives on healthy skin even when there's no active infection.
