Acetaminophen, Butalbital, Codeine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group White Pepper interacts with.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo: the codeine part of your medication is actually a prodrug. Your body has to use an enzyme called CYP2D6 to convert codeine into morphine, which is what gives you the real pain relief. Lab studies hint that piperine, the spicy compound in white pepper, may slow that same enzyme down a bit.
If CYP2D6 is blocked, less codeine turns into morphine, so theoretically you could get less pain relief from this medicine. The good news is that this is based only on test-tube research, not human studies, and the sprinkle of pepper on your dinner almost certainly won't matter. Only concentrated piperine supplements would be worth a chat with your pharmacist or doctor.
