Apalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Erleada (apalutamide): your liver uses an enzyme called CYP3A4 to help break it down. Piperine, the spicy compound in white pepper, can mildly block that same enzyme in lab studies. In theory, blocking it could let a bit more apalutamide build up, which might slightly bump up its side effects, things like fatigue, joint pain, rash, high blood pressure, or diarrhea.
The honest truth, though, is that this concern is theoretical and based on test-tube data, not real people. The sprinkle of white pepper on your dinner is very unlikely to matter. Concentrated piperine supplements are a different story. With a cancer medication like this, I'd just run any piperine supplement by your oncologist or pharmacist first.

What Is Apalutamide Used For?
Brand name: Erleada
Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.
Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
