Why this applies to Celecoxib, Tramadol

Celecoxib, Tramadol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that some constituents of white pepper inhibit cytochrome P450 2D6 (CYP2D6). Theoretically, concomitant use may affect drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Seglentis combines celecoxib with tramadol. Here's the part that matters: tramadol is a prodrug. Your liver enzyme CYP2D6 turns it into the form that actually relieves pain. Lab studies hint that piperine, the spicy compound in white pepper, may slow that same CYP2D6 enzyme. In theory, that could mean less of tramadol's active painkiller is formed, so you might get weaker pain relief than expected.

The good news is this is only a theoretical, test-tube finding, and the pepper you sprinkle on dinner is very unlikely to cause trouble. Concentrated piperine supplements are the bigger question mark. If you take one regularly and notice your pain isn't well controlled, just check in with your pharmacist or doctor before changing anything.

Celecoxib, Tramadol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the thing about Seglentis: it combines two medicines, and the piperine in white pepper can in theory slow down the liver enzyme (CYP3A4) that helps clear them. If that pathway gets a little blocked, more drug could hang around in your body, which may nudge up the usual side effects, things like drowsiness, dizziness, nausea, constipation, or stomach upset from the celecoxib portion.

Now, I want to keep this in perspective. This is a theoretical concern based on lab studies, not real patient cases, and the amount of pepper you'd sprinkle on dinner is very unlikely to matter. Concentrated piperine supplements are a different story. If you're considering one, just run it by your pharmacist or doctor first.
