Cobimetinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Your body clears cobimetinib (Cotellic) using a liver enzyme called CYP3A4. The piperine in white pepper has been shown in lab dishes to slow this same enzyme down. In theory, if that enzyme works slower, cobimetinib could build up a bit more in your system, which might push up the chance of side effects like diarrhea, nausea, skin rash, sun sensitivity, or changes the doctor watches for in your heart and eyes.

Here's the reassuring part: this is a theoretical concern based only on test-tube data, not real patient cases. Normal cooking amounts of white pepper on your food are very unlikely to matter. Concentrated piperine or black pepper extract supplements are the real question. Since cobimetinib is a serious cancer medicine, please run any pepper-based supplement past your oncology pharmacist or doctor first.

What Is Cobimetinib Used For?

Brand name: Cotellic

Cotellic (cobimetinib) is used in two ways. First, it treats adults with unresectable (cannot be surgically removed) or metastatic (spread to other parts of the body) melanoma that carries a BRAF V600E or V600K gene mutation — but only in combination with vemurafenib, another cancer drug. A tumor test must confirm the BRAF mutation before starting this combination.

Second, Cotellic is used on its own as a single agent to treat adults with histiocytic neoplasms, a group of rare cancers involving immune cells called histiocytes.
