Dextromethorphan, Guaifenesin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group White Pepper interacts with.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with these cough and congestion combos: the dextromethorphan part is cleared by a liver enzyme called CYP2D6. Lab studies hint that piperine, the spicy compound in white pepper, can slow that enzyme down. If that happened in a real person, dextromethorphan could build up a bit more than usual, which might mean extra drowsiness, dizziness, or a foggy, off feeling. The guaifenesin piece isn't really affected here.
The good news? This is purely a theoretical concern from test-tube data, and ordinary white pepper on your dinner is very unlikely to cause any trouble. I'd only pause on concentrated piperine supplements. If you take those regularly, it's worth a quick word with your pharmacist or doctor.

Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist with your cough and chest medicine. The dextromethorphan part of this combo is partly cleared by a liver enzyme called CYP3A4 (it also uses another enzyme, CYP2D6). Piperine, the spicy compound in white pepper, can slow down CYP3A4 in lab studies. In theory, that could let a little more dextromethorphan build up, which might mean more drowsiness, dizziness, or a foggy, lightheaded feeling. The guaifenesin side isn't really affected this way.
The good news: this is only theoretical, based on test-tube data, not real-world cases. Normal cooking amounts of white pepper are very unlikely to cause trouble. If you take concentrated piperine supplements, just mention it to your pharmacist or doctor before combining them.
