Dolasetron falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group White Pepper interacts with.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: dolasetron (Anzemet) is processed by a liver enzyme called CYP2D6, and lab studies hint that piperine, the spicy compound in white pepper, might slow that enzyme down. If that happened, dolasetron could clear a bit more slowly and build up. That could mean stronger effects, including the kind of side effects this drug can cause, like headache, dizziness, or changes in your heart's electrical rhythm (something doctors already watch for with this medicine).
That said, this is purely theoretical and comes from test-tube research, not real people. The amount of white pepper you'd sprinkle on food is very unlikely to matter. Concentrated piperine supplements are a different story, so check with your pharmacist or doctor before adding one.

Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist with dolasetron (Anzemet). Your body relies in part on an enzyme called CYP3A4 to break this medicine down. The piperine in white pepper has shown, in test-tube studies only, that it can slow that enzyme. If it slowed it in real life, a bit more dolasetron could linger in your system, which might nudge up its usual effects like headache, dizziness, tiredness, or constipation.
Honestly, this is a theoretical concern, not something proven in people, and the pepper you sprinkle on dinner is very unlikely to matter. Concentrated piperine supplements are a different story. If you're using one, just give your pharmacist or doctor a heads-up before mixing the two.
