Mexiletine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that some constituents of white pepper inhibit cytochrome P450 2D6 (CYP2D6). Theoretically, concomitant use may affect drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the gist for mexiletine: your liver leans heavily on an enzyme called CYP2D6 to break this drug down and clear it. Lab studies hint that piperine, the spicy compound in white pepper, can partially block that same enzyme. If it slows your clearance, mexiletine could build up in your blood, and since it's a heart rhythm drug with a fairly narrow safe range, even small increases matter. More drug on board can mean dizziness, nausea, tremor, blurred vision, or in rarer cases new rhythm problems.

The reassuring part: this is theoretical, based on test-tube work, not real patients. Normal cooking amounts of white pepper are very unlikely to cause trouble. Concentrated piperine supplements are the bigger question, so check with your pharmacist or doctor before using one.

What Is Mexiletine Used For?

Brand name: Mexitil

Mexiletine Hydrochloride capsules are indicated for the treatment of documented, life-threatening ventricular arrhythmias — for example, sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers). Because mexiletine itself can trigger arrhythmias, it is not recommended for minor or symptom-free irregular heartbeats.

Treatment is started in a hospital setting so heart rhythm can be monitored closely. Antiarrhythmic drugs, including mexiletine, have not been shown to improve overall survival in patients with ventricular arrhythmias.
