Why this applies to Remdesivir

Remdesivir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist for white pepper and remdesivir (Veklury). The thinking is that piperine, the spicy compound in pepper, may block a liver enzyme called CYP3A4. Since remdesivir is partly handled by that enzyme, slowing it down could in theory let a bit more drug build up, which might mean a higher chance of side effects like nausea, headache, or changes in liver enzyme blood tests.

That said, this is purely theoretical, based on lab dish studies, not real patients. Remdesivir is also given by IV in a hospital setting where you're closely watched. Normal cooking amounts of white pepper are very unlikely to cause any problem. If you take concentrated piperine supplements, just mention it to your care team so they're aware.

P-GLYCOPROTEIN SUBSTRATES

Remdesivir also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, white pepper might increase levels of p-glycoprotein substrates. The piperine constituent of white pepper seems to inhibit p-glycoprotein in vitro.
Some drugs metabolized by p-glycoprotein include some chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the situation with white pepper and remdesivir (Veklury). The piperine in pepper has, in lab dishes only, shown a slight ability to block a transporter called P-glycoprotein, which helps move certain drugs through the body. In theory, blocking it could let a little more remdesivir hang around.

I want to keep this in perspective. This is a theoretical concern based on test-tube work, not real people, and remdesivir is given as an IV in a hospital or supervised setting where you'd be monitored anyway. Everyday seasoning amounts of white pepper are very unlikely to matter. If you're considering concentrated piperine supplements, just mention it to your care team so they can keep an eye on things.

What Is Remdesivir Injection Used For?

Brand name: Veklury

Veklury (remdesivir) is given by IV infusion to treat COVID-19 caused by the SARS-CoV-2 virus. It is approved for adults and for children from birth through age 17 who weigh at least 1.5 kg (about 3.3 lbs).

Veklury can be used for patients who are hospitalized with COVID-19, or for patients who are not hospitalized but have mild-to-moderate COVID-19 and are at high risk of getting worse — including being hospitalized or dying from the disease.
