Trazodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that some constituents of white pepper inhibit cytochrome P450 2D6 (CYP2D6). Theoretically, concomitant use may affect drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the gist with white pepper and your trazodone. Trazodone is partly cleared by a liver enzyme called CYP2D6. Lab studies hint that piperine, the spicy compound in white pepper, can slow that enzyme down a bit. If that happened in your body, trazodone could clear a little slower and build up, which might make its effects stronger. With trazodone that usually means more drowsiness, dizziness, or feeling groggy.

The good news is this is only a theoretical concern based on test-tube research, not real patients. Sprinkling white pepper on your food is very unlikely to matter. Concentrated piperine supplements are where I'd be more cautious. If you're considering one, just run it by your pharmacist or doctor first.

Trazodone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the short version: your body uses an enzyme called CYP3A4 to help break down trazodone. Piperine, the spicy compound in white pepper, has shown in test-tube studies that it can slow that same enzyme down. If trazodone is cleared more slowly, a bit more of it can hang around in your system, which in theory could nudge up its usual effects, things like drowsiness, dizziness, dry mouth, or feeling lightheaded when you stand up.

The reassuring part is that this is only a theoretical concern based on lab work, not real patient reports. Sprinkling white pepper on your dinner is very unlikely to cause trouble. Concentrated piperine supplements are a different story, so chat with your pharmacist or doctor before using those alongside trazodone.
