Vincristine Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the basic idea: your body clears vincristine using a liver enzyme called CYP3A4. The piperine in white pepper has been shown, in lab dish studies, to slow that enzyme down. If that happened in a person, vincristine could clear more slowly and build up, which may mean stronger effects and more side effects. With vincristine that can look like nerve problems (tingling, numbness, weakness, constipation), low blood counts, or worse nausea.

The reassuring part is that this is theoretical, based on lab and animal work, not real patient cases. Normal cooking amounts of white pepper are very unlikely to matter. Still, vincristine is a powerful chemotherapy drug, so before using any concentrated pepper or piperine supplement, please run it by your oncology team or pharmacist first.

P-GLYCOPROTEIN SUBSTRATES

Vincristine Sulfate also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, white pepper might increase levels of p-glycoprotein substrates. The piperine constituent of white pepper seems to inhibit p-glycoprotein in vitro.
Some drugs metabolized by p-glycoprotein include some chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Vincristine is a powerful chemotherapy drug, and your body relies on a transporter called P-glycoprotein to help pump it out and keep its levels in check. The piperine in white pepper has been shown in lab dishes to slow that transporter down. In theory, that could let more vincristine build up in your system, which might mean stronger side effects like nerve tingling or numbness in the hands and feet, constipation, or low blood counts.

Here's the honest part: this concern comes from laboratory studies only, not from real people, so we don't truly know if it matters in everyday life. Normal cooking amounts of pepper are very unlikely to cause trouble. Concentrated piperine supplements are a different story. Since vincristine is serious medicine, please run any supplements past your oncology team first.
