Zolpidem falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group White Pepper interacts with.

In vitro evidence suggests that piperine, a constituent of white pepper, inhibits cytochrome P450 3A4 (CYP3A4). Theoretically, concomitant use with white pepper might increase the effects and side effects of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the simple version. Your body uses an enzyme called CYP3A4 to break down zolpidem and clear it out. The piperine in white pepper may slow that enzyme down a bit. If the enzyme works slower, more zolpidem can hang around in your system, so the effects could feel a little stronger than usual.

For a sleep medicine, that means you might notice more drowsiness, dizziness, a groggy feeling the next morning, or trouble staying steady on your feet at night. Honestly though, this is a theoretical concern based on lab studies, not real patients. Normal cooking amounts of white pepper are very unlikely to matter. Concentrated piperine supplements are the bigger question, so check with your pharmacist or doctor before pairing them.

What Is Zolpidem Used For?

Brand names: Ambien CR, Intermezzo, Sublinox

Zolpidem is used short-term for insomnia. Most formulations — including Ambien, oral tablets, and Zolpidem Tartrate Capsules — are approved to help people who have trouble falling asleep at bedtime. Zolpidem Tartrate Capsules are specifically indicated for adults under 65.

The extended-release formulations (including Zolpidem Tartrate Extended-Release Tablets) help with both falling asleep and staying asleep through the night. Sublingual tablets (Edluar, Zolpidem Tartrate Sublingual Tablet, and Zolpidem tartrate sublingual) are taken as needed when you wake up in the middle of the night and cannot get back to sleep — but only if at least 4 hours of sleep time remain before you need to be up.
