Elexacaftor, Tezacaftor, Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Wild Cherry interacts with.

In vitro research suggests that wild cherry can inhibit cytochrome P450 3A4 (CYP3A4) enzymes. Theoretically, wild cherry might increase levels of drugs metabolized by CYP3A4. However, so far, this interaction has not been reported in humans.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and others.

Here's the quick picture with Trikafta. All three pieces of this medicine (elexacaftor, tezacaftor, and ivacaftor) are cleared by a liver enzyme called CYP3A4. Lab studies hint that wild cherry may slow that enzyme down. If that happened in real life, your body could break Trikafta down a little less, letting levels creep up. That might mean more of the side effects this combo is known for, like headache, belly pain, diarrhea, a rash, or changes on liver blood tests.

I want to be honest, though: this is only a test-tube finding, and it has never been reported in actual patients. Normal cough-syrup or lozenge amounts of wild cherry are unlikely to cause trouble. Still, since Trikafta matters so much for your lungs, run any wild cherry product by your pharmacist or CF team first.

Wild cherry bark is a traditional cough remedy found in many old-fashioned cough syrups and lozenges, but there is little modern scientific evidence proving it works. The bark, leaves, and seeds naturally contain cyanide-producing compounds, so it should be used only in properly prepared products and avoided in large amounts. What Is Elexacaftor, Tezacaftor, and Ivacaftor Used For?

Brand name: Trikafta

Trikafta (elexacaftor, tezacaftor, and ivacaftor) is used to treat cystic fibrosis (CF) in adults and children aged 2 years and older who have a clinical diagnosis of CF and carry at least one CFTR gene variant that is expected to respond to the medication — either based on clinical trial data or laboratory testing.

If a patient's genetic variant is not already known, an FDA-cleared CF genetic test should be used to confirm eligibility before starting treatment.
