Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains 2 active ingredients: acetyl-L-carnitine hydrochloride and acetyl-L-carnitine arginate dihydrochloride. Both are forms of acetyl-L-carnitine, a compound that supports cellular energy production and brain function.
The capsule also contains inactive ingredients—cellulose, microcrystalline cellulose, vegetable stearate, and silica—which serve as binders and fillers.
Does it work?
Moderate evidence
According to our data, acetyl-L-carnitine is rated possibly effective for age-related cognitive decline, Alzheimer disease, alcohol use disorder, diabetic neuropathy, and depression. That means there's some evidence it may help, but the research isn't yet strong enough to call it proven.
If you're considering it for one of these uses, talk with your doctor about whether it makes sense for your situation.
How safe is it?
Well-documented data
Acetyl-L-carnitine is generally well tolerated in the short term, but long-term safety hasn't been fully established, so it's worth checking in with your pharmacist if you're planning to use it long-term. The most common side effects are agitation, dry mouth, headache, insomnia, and reduced appetite.
Some people also report a fishy odor in urine, breath, or sweat. Nausea, vomiting, and digestive upset can occur, though they don't happen more often than in people taking placebo.
If you're pregnant, there isn't enough safety data to know either way—talk with your doctor. The safety data advises against use while breastfeeding.
Meds to double-check
Moderate interaction found
Before you take this product, check your medications against these types: serotonergic drugs (like SSRIs and SNRIs), blood thinners such as warfarin and acenocoumarol, and thyroid hormone replacement. All three carry a Moderate interaction risk.
Use the search tool on this page to look up your exact medications.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
Acetyl-L-carnitine may be worth considering if you're interested in supporting cognition or managing conditions like diabetic nerve pain, but it's not without cautions. If you take serotonergic antidepressants, blood thinners, or thyroid hormone, you'll need to check with your pharmacist before starting.
Long-term safety isn't fully established, so this is a conversation to have with your doctor rather than a solo decision.

Acetyl-L-Carnitine Arginate by Life Extension Drug Interactions
HelloPharmacist Interaction Report
Acetyl-L-Carnitine Arginate by Life Extension does interact with several medication types.
The most serious concern is with serotonergic drugs (antidepressants, certain pain medications, and others that affect serotonin). Animal research shows acetyl-L-carnitine can raise serotonin levels in the brain, and theoretically this could increase the risk of serotonin syndrome and blood vessel constriction issues when combined with these medications.
Read the full breakdown — every affected drug type, severity by severity
You'll also want to flag blood thinners (anticoagulants). Acetyl-L-carnitine may increase the anticoagulant effects of warfarin and acenocoumarol, which could raise your bleeding risk.
Thyroid hormone replacement is another type to watch: acetyl-L-carnitine might decrease how well it works by blocking thyroid hormone from entering cells where it's needed.
All four of these interaction types carry a Moderate severity rating in our data. Altogether, these interactions span 203 individual medications.
Before you start this product, run your exact medications through the search tool on this page to see if any of yours are affected.

Acetyl-L-Carnitine Hydrochloride
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine, the parent compound of acetyl-L-carnitine, might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with acetyl-L-carnitine and warfarin.
