Lactobacillus acidophilus is generally well tolerated in healthy adults and children. That said, because it's a live microorganism, there is some concern it could cause infection in certain patients—cases of bacteremia (bacteria in the blood), sepsis, abscesses, and other serious infections have been reported, though most are thought to happen when bacteria from the gut cross into areas where they don't belong.

Gastrointestinal side effects like gas, bloating, abdominal pain, and diarrhea are uncommon but possible, especially when combined with other probiotics. Rash and itching have been reported in a small number of trial participants, though it's unclear if L. acidophilus was the cause.

Antibiotic Drugs

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
