Short-term use of andrographis is generally well tolerated, though long-term safety and product quality aren't well established, so use with caution. Mild side effects can include stomach discomfort, altered taste, diarrhea, dizziness, fatigue, headache, nausea, vomiting, itching, and rash.

These are more likely at higher doses (5–10 mg/kg of the active compound andrographolide) or beyond 14 days of use. Serious allergic reactions, including anaphylaxis, are rare but have been reported.

Skin reactions—including severe ones like Stevens-Johnson syndrome—have also occurred. Do not use andrographis during pregnancy; animal studies and theory suggest it may harm reproduction.

There isn't enough safety data for breastfeeding, so avoid it then too.

However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
