B. bifidum is generally well tolerated in healthy people. That said, if you have a weakened immune system, talk with your doctor first — the evidence is limited and these organisms haven't been studied extensively in that population.

Bloating and flatulence are reported with probiotic use in general, though they haven't been specifically tied to B. bifidum. One case of vomiting and fever was reported in a child taking B. bifidum with another probiotic, but it's unclear whether the probiotics caused it.

Very rare cases of Bifidobacterium sepsis have occurred in critically ill patients, though not from B. bifidum specifically.

Since B. bifidum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. bifidum preparations by at least 2 hours.
