Allergic reactions — including rash, itching, or hives — are rare but can happen; they've been theoretically linked to the cobalt in the B12 molecule. Injection site reactions are the most common adverse effect when B12 is given by injection, not by mouth.

A very rare complication from correcting severe B12 deficiency is a dangerous drop in potassium (low blood potassium), which can be serious — if you have severe deficiency, your doctor should monitor you closely. B12 is considered safe during pregnancy and breastfeeding at normal recommended amounts; follow your provider's guidance.
