Together they're meant to support stomach acid and protein digestion. The capsule also contains four inactive ingredients—gelatin, magnesium stearate, silica, and microcrystalline cellulose—which are standard fillers and binders used to form and stabilize the capsule.

Betaine hydrochloride can irritate your stomach and is not safe if you have ulcers, gastritis, or true acid reflux—the very thing this ingredient makes worse. It may cause heartburn, and theoretically it could irritate or slow healing of ulcers.
Pepsin is generally well tolerated for most adults, though some people may experience digestive upset or allergic reactions. For pregnancy and breastfeeding, the safety data isn't there.
Betaine HCl should be avoided during pregnancy and breastfeeding because there isn't enough information to know it's safe. Pepsin also lacks established safety during pregnancy and should be avoided unless a doctor prescribes it; breastfeeding safety is not well studied, so medical advice is needed before use.

Betaine is a source of hydrochloric acid, or stomach acid. Betaine HCl supplement is typically used to increase levels of hydrochloric acid in the stomach. This formula also contains pepsin, a digestive enzyme that assists with the digestion of protein.
